Welcome to Heartland Health Care Center, located in the scenic NW hills of Austin. Heartland Health Care Center provides Post Acute Rehab,  Assisted Living, & Long Term Care.

Heartland prides itself on a proven record of service with two deficiency free state surveys in 2010 and 2011. Since 1989, Heartland  Health Care Center has been serving the community offering services ranging from assisted living, rehabilitation/post hospital care and long  term care. Heartland holds a special attraction for couples that require differing levels of assistance and medical care. Our facility also has a beautiful enclosed courtyard with a gazebo and is a pet friendly facility.   We also have a full service salon to help our clients look as good as they feel.

Post Acute Care Services Offered:

After surgery, illness or injury, you want to get back to your life as quickly as possible. Our post-hospital centers are your best way home after a hospital stay. We offer complex medical and rehabilitation services delivered by a team of professionals. We focus on caring for patients with many conditions including, but not limited to:

• Orthopedic care including joint replacement, injuries and amputation
• I.V. therapy
• Stroke and neurological care
• Post-surgery recovery
• Wound care
• Trach care
• Cardiac care
• Diabetes management
• Hospice care
• Respite or vacation care
• Special diets
• Ambulation and gait training
• Wii Therapy

Individualized patient care plans are designed through an interdisciplinary team approach to determine the needs and meet the goals of our patients.  Our advanced modalities program features an array of treatments including electrical stimulation, ultrasound and or short-wave diathermy.  These modalities can assist and treat wounds, muscle spasms, pain, edema, joint stiffness and contractures. The Heartland therapy team will work with you for a safe discharge home by performing home evaluations and working with you on your community reentry goals.  Our social services staff will help you plan any additional support you may need once you are home.  The team of dedicated professionals includes; medical director, nurse practitioner,  licensed nurses, certified nursing assistants, licensed in-house therapists/aids, dietary manager, and licensed social workers.

Along with Medicare and any Medicare replacement plans, Heartland Health Care is proud to be contracted with the following insurances; Aetna, Amerigoup, Blue Cross Blue Shield, Care Improvement Plus, Humana, TriCare Humana, PHCS, Pacificare/Secure Horizons, Texas True Choice, United HealthCare and others.  

Assisted Living

The ground level at Heartland Health Care center is a Type B assisted living facility. There are community rooms, a full service dining room, beauty salon and an laundry room.  Services provided include; medication management, three meals a day in our dinning room, housekeeping services, three times a week shower assistance, planned activities and outings with additional services optional for purchase as needed.

Each of the rooms comes equipped with two call lights for residents' connivance in receiving assistance from the 24 hour staff. Podiatrist, ophthalmologist, dentist, mobile x-ray, and primary care physician visits are all services that can be done in-house.

Long Term Care

HCR Manor Care offers 24-hour nursing care to chronically ill and frail elderly patients. Long term care patients can partake in activities geared towards their cognitive and physical abilities. Our team of experienced professionals, including registered/licensed nurses, certified nursing assistants, dietitian, licensed therapists and therapy assistants, is focused on providing services prescribed by physicians. We do permit various hospice companies to administer services.

Our center is also accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO).
