Care-Connect At Home is a professional home care service for seniors. We are a member of the Lutheran Social Services of the South, Inc. family of ministries, a non-profit agency providing help, healing, and hope to children, seniors, and disaster victims in need. Care Connect Helping Seniors Services.

Care-Connect is the one-stop resource seniors and families need to gather information, implement careand monitor the care needed to live safely at home.

Our comprehensive assessment provides a snap-shot of services needed today, and helps you plan for future care. Our experts relieve your stress, time and the costs associated with research and management of senior care providers.

How It Works:

Our Geriatric Care Manager...

1. completes a comprehensive assessment
2. connects the client to needed services
3. ensures all services are in place
4. monitors those services
5. reports to out-of-town family members if requested
6. will continue services as long as needed, including facilityplacement and facility advocacy if that is necessary.

About Sara Rodriguez, RN

Sara Rodriguez, RN has been an RN for over 19 years, with 15 years experience in long-term care. Rodriguez has been a Director of Nursing for Skilled Nursing Facilities, an RN Nurse Consultant for Assisted Living Facilities and Skilled Nursing Facilities and an RN Nurse Manager for Hospice. She has worked in mulitple long-term care roles at Lutheran Social Services in her five years with the agency.
