Skilled Nursing Services

Medicare-covered skilled nursing services may include, but are not limited to:

• Diabetes care and management
• Infection prevention
• Observation and assessment
• Education and training activities
• Wound care
• Total Joint Rehabilitation Program
• Cardiac Disease Management
• COPD Management
• CHF Management
• Pain management
• Medication administration per Medicare guidelines
• Evaluation of need for special devices and equipment for more mobility

Diabetes Management

We offer an extensive range of specific diabetes services. By continuing to educate our patients and ourselves, we're able to stay on the leading edge of quality diabetes care.

Our program includes:

• Management of any short and long-term complications
• Proper drawing techniques for blood sugar analysis and insulin administration
• Nutritional management

Diabetes with Acute Complications

Our care includes a complete evaluation process of a patient's physiological functions, environmental factors and cognitive abilities. We make sure the patient or family member is able to maintain stable blood sugar levels in order to prevent or control long-term and severe complications.

Diabetes with Chronic Complications

We offer skilled nursing provided in daily visits (more if recommended by the physician) when a patient is unable to consistently self-manage the disease. In such cases, our services may include:

• Insulin administration
• Continued education and observation
• Blood sugar monitoring
• Treatment and medication assessment

Monitoring for long-term complications

• Physical and occupational therapy
• A blood glucose monitor and/or non-routine supplies may be covered if a skilled nurse is administering insulin to a patient
• Finger-stick supplies may be covered if a patient is insulin-treated and documentation supports medical necessity or indicates an unstable diabetic condition
• Pre-filled syringes are covered if a patient first qualifies for some other skilled nursing service

Skilled Therapy

Our Encompass Home Health skilled therapists work with patients and physicians to develop and implement individualized therapy programs to improve quality of life for our patients. Whether it's improving our patients' mobility or helping them communicate better, we've got it covered.

Physical therapy offers:

• Restored balance
• Improved mobility
• Reduced risk of falling

Occupational therapy provides:

• Improvement in daily tasks and essential activities of daily living
• Recommendations and fittings of self-help orthotic devices
• Creation of a safe and secure home environment

Speech therapy includes:

• Help with cognitive abilities

Additional Services

Home Health Aide

• Home health aide services are covered if a patient first qualifies for some other skilled service
• The frequency and duration of home health aide services are dependent on a patient's functional status

Medical Social Work

• Medical social work services are covered if a patient first qualifies for some other skilled service
