Hill Country Care Providers Austin Home Health and In Home Care Agency 

Hill Country Care Providers (HCCP) is a Geriatric Care Management service, licensed private-pay medical home health agency and a non-medical personal attendant service.

 

We can help you manage the care of your loved ones while enhancing their quality of life.  We are Texas licensed and certified.

GERIATRIC CARE MANAGEMENT

The challenges of having to make health and life decisions for a parent, spouse or older relative can be overwhelming. Where does a family member start when they have not had to make decisions for an older member of their family before?

When you don’t know where to begin, the services of a Geriatric Care Manager can be invaluable in determining the needs of your loved one and finding resources in your community to meet those needs.

Hill Country Care Providers staff includes an RN certified as a Geriatric Care Manager by the National Association of Professional Geriatric Care Managers and a Gerontologist as well nurses who are professionals with specialized knowledge and experience in senior care issues. We will work with you to develop and implement a specifically suited plan of care.

Our geriatric care managers collaborate with you on the initial consultation and if you wish, we can continue your loved one's care with follow-up services.

Geriatric Care Management includes:

It is our belief that our clients benefit greatly when they can stay in familiar surroundings while still receiving superior care and medical supervision.

Hill Country Care Providers offers services to older and disabled adults who are unable to perform tasks associated with daily living to allow them to remain as independent as possible and in their own homes or other familiar environment.

Individuals who struggle with age related diseases such as dementia, Alzheimer’s, Parkinson’s, cardiac diseases, diabetes, arthritis, and stroke benefit from our services improving the quality of their lives wherever they live.

Our services include assistance with the following:

Our caregivers are available from four to twenty-four hours a day or live-in.

Jackie Verdoorn
Founder

Founded in 2001, Hill Country Care Providers is locally owned and operated by Jackie Verdoorn, RN, BSN, MSN.

Jackie is a certified Professional Geriatric Care Manager with the National Association of Professional Geriatric Care Managers.

Jackie's commitment to providing in-home care is based on her own personal experiences caring for her parents and from many years assessing needs, finding appropriate resources, and providing hands-on medical and non-medical care.

Jackie realized early in her quest for reliable caregivers that her standards for experience and quality of care went beyond what was currently available among private caregivers and personal assistance agencies. She developed her own agency of experienced caregivers, many of whom have been with Hill Country Care Providers since its inception.

Hill Country Care Providers includes a multidisciplinary team of RNs, LVNs, gerontologist, caregivers, and an MD medical director for geriatric care management, private-pay medical and non-medical in-home services.

Hill Country Care Providers dedicates a great deal of time and energy towards hiring the most qualified individuals who will uphold professional standards, meet our high expectations and provide the level of caring and expertise you and your loved ones deserve. 

We are happy to introduce to you Tam Cummings, M.S., Gerontologist as a new member of our team. She has served as director of Alzheimer’s units in assisted living communities and skilled facilities as well as having assumed leadership roles as a social worker in long-term care settings.

With Tam’s expertise, Hill Country Care Providers can now expand our geriatric care management capabilities.  She has the most current assessment and testing tools available to identify specific care needs, allowing HCCP to offer families real solutions for continuing care. 

In addition, our caregiver training is being upgraded to include Tam’s extensive knowledge of dementia as well as offering a valuable new community resource to individuals and community outreach programs unequaled in Austin.

Tam has recently completed a book entitled “A Guide to Understanding Alzheimer’s Disease and Other Dementias: Physiology, Perceptions, Communication, Stages, Grief and Death. She is currently writing two other books on grieving and activities for persons with dementia.
