Outreach Health Services is a leading provider of health care services and support to individuals, families, and the health care community. Outreach Health Services offers home health services to clients of all ages through these payer sources: Medicare, Medicaid and Insurance/Private Pay. Outreach offers a variety of services in many areas across the state of Texas including:
In addition to Medicare and Medicaid home health services,
Outreach Health Services provides skilled home care and support services to
persons who pay for the services We employ nurses, physical therapists, occupational therapists, speech therapists, home health aides and medical social workers, plus homemaker-companions who assist patients with personal care and daily living activities. Outreach Health Services currently has statewide and regional contracts with various managed care organizations as well as other third-party payers. As different opportunities become available, we have adapted services and hired qualified staff to fit the needs of our patients and our payer sources. All services are available on an hourly or per-visit basis. For insurance cases, Outreach Health Services will verify the patient's coverage, including any co-payment, and bill the insurance company or other third-party payer for the patient. HOME

Approved Outreach Health Services is meeting the growing demand for superior, short-term, intermittent care through its Medicare Services Division. Under a plan of care and orders from a client's physician, Outreach provides a broad spectrum of skilled home health services:

HEALTH SERVICES: Medicaid
Eligible Some home health services are covered by Medicaid, which is
a state-funded program that provides nursing services and home health aides for
qualified individuals. To qualify for Medicaid home health services, the patient must be confined to the home due to illness or injury, and leaving the home must require a considerable and taxing effort, assistance of another and/or assistive devices. Services must be ordered by a physician, be reasonable and necessary to the treatment of the injury or illness and initially require a nurse, physical therapist or speech therapist. To see if you qualify for Medicaid services, contact your local Texas Department of Human Services office for eligibility information.COMMUNITY
CARE SERVICES DIVISION Additionally, Outreach also offers the Community Care Services Division which provides a wide range of services to the elderly and disabled that help them preserve their dignity by maintaining their independence at home. These services are provided through the Texas Department of Aging and Disability Services (DADS) to individuals who meet income and Medicaid eligibility requirements under the CCAS (Community Care for the Aged and Disabled) and CBA (Community Based Alternatives) programs and include: Primary Home Care / Family Care (PHC/FC) - Persons whose health problems cause them to be functionally limited in performing activities of daily living can benefit from our Primary Home Care/Family Care services. PHC provides non-technical, personal care tasks such as: bathing, dressing, exercising, routine hair and skin care, toileting, transfer/ambulation, meal preparation and assisting with self-administered medications. Services also can include home care management such as cleaning, laundry and shopping, escort to doctors appointments and other medical treatments. An initial physician's order is required for participation in this program. LifeBox24.7 - Emergency Response Services (ERS) Wouldn’t it be a great comfort to be able to live independently, knowing that you are safe and secure in your home and that help can be there when you need it? Receiving help is as simple as pushing a button. LifeBox24.7, a private pay ERS device, provides instant connection for help whenever it is needed 24 hours a day, 7 days a week.
Community Based Alternatives (CBA) - This program provides home and community-based services to people who are elderly and to adults with disabilities as a cost-effective alternative to living in a nursing home. DADS employees provide case management services. Elderly and disabled clients can receive attendant services, nursing care and therapies through this program. Medical supplies, durable medical equipment and home modifications also can be provided through CBA. A physician's order is needed for participation in this program.
