Every new client gets a RN Care Coordinator who is a registered nurse or a certified geriatric care professional. This is your single point of contact for all issues—no need to call the office and talk to a different person every time you have a question or concern.
Your coordinator is in constant communication with all caregivers who receive a general briefing and specific care instructions prior to assisting each new client.

Transitioning Home Program
RN Care Coordinators
An important part of transitioning home hinges on all parties involved, including social workers and therapists, clearly communicating regarding your care needs. 2nd Family will coordinate the communication between all parties, keeping everyone updated concerning recovery and progress updates.
Your transition will be managed directly by a dedicated RN Care Coordinator who will oversee all aspects of your care, from crafting the plan of care to supervising the caregivers and managing your transition home.
When your loved one is returning home from the hospital or skilled nursing facility, the transition can be stressful. More often than not, the senior is still dealing with some effects of the injury or medical condition. This can make adjusting to daily life challenging, and cause increased difficulty in performing basic activities of daily living.

Certified Caregivers
All of our caregivers are W-2 employees who have a valid CNA or GNA
certificate and are all bonded and insured through our company. They are
highly trained and registered with the state licensing agency and has
undergone both classroom and clinical training.
Additionally, we train our staff to specialize in Alzheimer’s and Dementia care using materials certified by the Alzheimer’s Association. Our combination of experience and education ensure that we provide only…the best care.
